    If you don't know yet, no need to go thru an agency anymore for DFA authentication of documents , these agencies usually try to rip you off! go to DHL office at Siliman Ave ,surrender your document and after filling a form and paying 500 pesos only, after around 10 days only,you will be called to retrieve your document.the price per document even goes down if you have many of them to authenticate .:smile:

    Below are Requirements for DFA Authentication:

    1. Birth/Marriage/Death Certificate or Certificate of No Marriage Record
    • It must be in security paper issued by NSO or must have been certified by NSO
    2. Transcript of Records and Diploma (Collegiate Level)
    • Certified true copies from the school
    • Secure Certificate of Authentication and verification from CHED
    3.Transcript of Records and Diploma for state college and universities
    • Certified true copies from the school
    • Secure Certificate of Authentication and Verification from the school signed by the school/university registrar
    4.Transcript of Records and Diploma for Technical or vocational courses
    • Certified true copies from the school
    • Secure Certificate of Authentication and Verification from TESDA
    5.Form 137 and Diploma (Highschool and elementary level)
    • Certified true copies from the school
    • School principal’s certification
    • Division superintendent’s endorsement to Dep-Ed Regional Office
    • Certificate of Authentication and Verification from Dep-Ed Regional Office
    6.Medical/AIDS free Certificate
    • Authenticated by the DOH (applicable to the f.f Countries: Spain, Palau, Libya, Oman, Cuba, Portugal, Greece, Cyprus and Angola, if not included submit the same requirements stated in #10
    7. CAAP issued license (authenticated)

    8. Driver’s Licensees
    • Secure certification from LTO
    9. Professional Licenses/Board Certificates/Board Ratings/Certifications
    • Certified true copies must be authenticated by PRC
    10. Certificate of Employment/Trainings/Seminars, baptismal certificate and other documents issued by a private entity.
    • Applicant must first secure an affidavit stating necessary factual circumstances and indicating certificates as annex or attachment
    • Affidavit must be notarized
    • Applicant must secure Certificate of Authority for a Notarial Act (CANA) Signed by the executive judge or the vice executive judge from the Regional Trial court which issued the Commission of the Notary Public
    11. Other notarized documents (SPA/Affidavit of Consent/Invitation/Guarantee/MOA/etc.)
    • After document is notarized, applicants must secure certificate of Authority for a Notarial Act (CANA) signed by the executive judge from the Regional Trial Court which issued the Commission of the Notary Public
    12. Court Decisions/Resolutions/Orders
    • Applicant must present certified true copies of the decision resolution or order
    • Applicant must secure copy of specimen signature of the court personnel who signed the certified copies from the Office of the administrative Services (Supreme Court)
    13. Immigration Records
    • Certified/Authenticated by the Bureau of Immigration
    14. DSWD Clearance
    • Travel Clearances for minors directly issued by the department
    15. NBI Clearances
    • NBI Clearances travel abroad must be issued by the NBI

    16. Police Clearances
    • Must be signed by the Chief of Police issued by the PBP in various police stations nationwide, usually by the police precinct which has jurisdiction over the applicants place of residence or applicant may opt to secure police certification from camp crame
    17. Barangay Clearances
    • Clearances issued by the barangay which has jurisdiction over the applicant’s place of residence and must have been authenticated by the office of the mayor which has jurisdiction over the barangay
    18.Export Documents
    • Must be authenticated by PCCI, DOH, DA or by BFAD
    19. Business Registration and Other Documents issued by a Government agency
    • True copy from the issuing office
    20. Foreign Documents
    • A Philippine Embassy or Philippine Consulate General in the country from where the document originated or by the said country’s Embassy or Consulate General based in the Philippines must have authenticated these documents

    DFA Authentication Procedure:

    1. Fill out the application form. You can download it here: https://www.formsphilippines.com/viewform/270/authentication-form
    2.Present a valid ID upon submission of the documents to the processing window
    3. Pay appropriate authetication fee (100 php per doc)
    4. Claim the authenticated document on the release date
