    I will soon do my Annual Report 2014 in Manila or Cebu.
    What Requirements Bi need, and what is the Procedure.

    I have already printing out the Annual Report 2014 Form (4 pages), but can anyone tell me,
    see page 3, which parents Father's name and Mother's name the BI asked for.
    I'm foreigner and have Permanent Visa 13 (a) and my wife is a Filipina.
    We have no children.
    My parents are died since long time ago, only my Filipina wife has living parents,
    so which father's name and mother's name BI asked for?.
