Yes McDonalds on Perdices street wants that little piece of paper filed in before entering the restaurant. They only ask for name, address and cell number.
It is all about the City Health Contact Tracing. That is why when you go in some places they want you to remove your mask and look at the camera. YNot had the sign in sheet but did not require the phone number, they had an inspection and were told to require that too, so now you have to give that info.
