I have always flown direct but I wonder if anyone has experience of long stop-overs: Is it permitted to leave the airport (I realise that this could only be allowed if the country has free entry or visa-on-arrival for the person's citizenship or an appropriate Visa has been obtained)?

    I don't know.
I had a 11 hr stop over in KL airport in Jan 2020. I just did a 3 month stay and on the way back home.
I was annoyed because Air Asia changed the flight times while I was a away otherwise would have never booked the flight with that stop over.
It was Jan 30th 2020, I was having a beer and filling in time reading internet news about Covid, plenty of people with masks on in the airport.

I thought to myself this Covid wont end well and went into my superannuation account and changed the risk to cash.

Was the best stop over I have had.

I once had an overnight stopover in Guangzhou China. The Chinese airline canceled the flight I was supposed to board so I had to stay in China for one day. They paid for the hotel room. Although economically the flight was a great deal the experience was not worth the savings.

The area of Guangzhou that I was in was a dirty industrial city.

The Chinese will give you a 24 hour visa on arrival for situations like this.

So this was a forced situation and you had no choice.

I am thinking more of a situation in which the passenger wanted to explore the local area (assuming the airport is not 100 miles way from human habitation!) and if the airline and/or immigration refused to let him leave the airport and he has to remain in the airport lounge until his connecting flight takes place.

I don't explore anymore but most of the time even though I usually fly premium economy, I book a long layover and stay at the airport hotel overnight just to make the trip easier. For me Taipei is a 5 hour layover and Inchon is 7 as the shortest available layovers, but as I say, I book a flight with a longer layover. I usually don't fly through the other cities anymore because of better experiences in Taiwan and Korea.

It varies on based on airline policy. Generally, if too long they will force you to book as two separate segments, which is what you are trying to avoid to save money.

Many years ago I did stop overs in Bangkok on the way to Guangzhou China. Flight landed around BKK 4pm and my Guangzhou connection was at 10am the next day.
First few trips I just hung out in my hotel right up the end of Soi 18 suk.
Didn't know anything about Bangkok back then. One trip I met this Japanese dude in my hotel bar and he took me to this place called Soi Cowboy........I was young and single back then what a eye opener.
The wife and I both love Bangkok and these days often do a few days stop over if we are traveling through BKK to Chaing mai.

You must be wary of the countries current covid policy regarding foreigners, so first I guess you must choose where you would like to transit and the local policies regarding hotels accepting foreigners, but only if immigration allow you to leave, and then the acceptance of your final destination accepting the fact that you did indeed overnight in another country that they might frown upon and hold it against you

there is strictly a no visa on arrival policy in china now and you will not be allowed to use the airport as a transit stop, not a snowballs chance

I understand that and it is a current issue (perhaps longer lasting) that complicates matters - but I doubt any airline will carry passengers for many years to come without approved vaccination documents and/or test results.
