You need to be more specific, is this for a property or chattel that you have for lease. You can go online and find templates according to your needs. I did this when selling a condo and it made life much easier, as there were no questions left unanswered which could have delayed the sale. Always include philippines in your search, as laws change from country to country.

Possibly you could turn the downloaded forms into PDF files and modify them before you print them. White out and initial would work too since you are going to need to have them notarized anyway.
Try and ask at the office of a "notary public". While none of my rental contracts so far in the phils have been notarized, I think they do have common contracts like this.
I guess it is situational. If it would benefit me, I would make sure that any agreement was notarized so that the other party could not say that wasn't their signature and they had never seen that document in their life. If I was worried about being held to a contract, I would be more than fine if it wasn't notarized. Have to remember where you are, you have to keep receipts for everything because all documents are assumed to be forgeries unless you can prove they are not. If it's worth making a contract, it's worth making a contract you can be reasonably certain will stand, or what is the point?
yeah of course it depends. If buying a used car it's absolutely necessary since otherwise you cannot transfer ownership at the lto. But when I'm renting an apartment here I don't see what's the point - exactly because personally I wouldn't need a contract at all and in fact I don't have one at the moment. I pay rent as long as I stay there, I stop paying rent the moment I move out. A contract could only put additional burdens on me (minimum stay etc), not in my interest to notarize this.
I finally got the original straightened out and found one simple agreement online...had to open an bank account here and this was a document they wanted and notarized unlike the U S Banks here...they do not have notaries and boy was a little bit of effort to find a lawyer also TY for your replys
