From memory; your Filipina Wife will need to supply a copy of, and the original of, her original Birth Certificate. She will need to apply for this as proof that she is a Filipino Citizen born in the Philippines. Birth Certificate | Philippine Statistics Authority
I am a bit lost and need some help......................went to the above website.............registered and then filled the online application form and had my info saved then up loaded a pic of my self that was asked for...........this was about a week ago and still no reply................have tried texting and ringing the 3 contact numbers on the website but no reply...........any advice what to do.....?? or any other advise on how to do NBI online......??
Many are having a problem with this, there is something on a Form you sent that tells them you are a foreigner & there is a different Set of rules for Foreigners SO I would suggest that you go to the NBI by the LTO and tell the Officer at the Front by the Windows what has happened, he will help you no end BTW go as early as you can 7.30 and they start at 8
Once you apply you will need to have paid and got an appointment time, when you pay you get a receipt with a number on it which you show when you get to nbi. They never answer emails.
When you go to the NBI Office, they will finger print you and put you in their system, they will send you a little ways back up the road to the shop with some fixers who will log online for you and register you, you then go to LBC and pay the fee, return to the NBI Office and show your Receipt from LBC, we paid a fee to the NBI to speed the process up as we needed my clearance in a shorter period of time... The guy from the NBI will then text you when your clearance is ready to be picked up. The NBI/ Fixers and LBC are all within walking distance of each other or there are tricycles often doing the rounds who can take you to the LBC out on the main road and back again once you are done with your payment...
I am planning to get my 13A visa in San Francisco prior to leaving the US. They do require a physical exam, complete set of tests including a chest X-ray. I will be bringing all those medical documents and other documents and forms to the SF Phil. Embassy in early July. They say it is a 2-day process. First day submit the documents and forms and have an interview. Second day return to pick up the visa. I believe the total cost will be $50 USD.
I had all the medicals done then some to be on the safe side, when I switched to using the Agent they told me I did not need any medicals, both times I went to Cebu, not once did they ask for any medical proof like what it states on the web site... The Agent was 100% correct!
