    BPI required the "health declaration form." Had to fill it out at the door, thinking of making some generic pre-filled out ones...

    did i see passing mc donalds (the older one), you have to fill out your details if wanting to sit down, its on printed mc donalds paper, there is a desk outside with a pen, bunch of names addresses phone numbers

    Yes McDonalds on Perdices street wants that little piece of paper filed in before entering the restaurant. They only ask for name, address and cell number.

    It is all about the City Health Contact Tracing. That is why when you go in some places they want you to remove your mask and look at the camera.

    YNot had the sign in sheet but did not require the phone number, they had an inspection and were told to require that too, so now you have to give that info.
