that was also my question and not answered yet..about notarization...
another question is, do we have to submit our passport with the Application Form and sent it all together
    with the Official to Cebu?
coz years ago my passport was lost in the BI system when I applied for permanent visa...
after more then 3 years the passport was found, it stucked in the BI Cebu Vault with all my Documents...they had
my Address but didn't bother to contact me...
I don't want that happened again...

    1 bring 2 2x2 pictures
2 fill in as much of the form as you can.
3 do not get it notorized, they will do this.
4 they will finger print on Saturday.
5 this person works in the bi at Cebu.
6 php 1500 imcludes all. Unless you have an overstay or did not do annual report last ur. Contact me if this has happened.
7 my wife will fly to Cebu when the official reciepts are ready. She will need all names and cell numbers on sat. And php 100 for expenses.
8 that's all, if you're not happy with this make a trip to Cebu or Manila.
And bring your passport and iCard with last official receipt.

As far as I know you will not need to send passport or iCard along with the papers.

It would be even better if they got a new office with aircon and all new equipment! There are enough expats here now to warrant this! No more piles of files and papers stacked everywhere and a sofa you can get up from and still feel good  if they want to attract foreigners they do need to get their act together and show everyone they can do this in the 21st century and not the 1930's

There has been a change to the annual ACR reporting, Saturday at Casablanca meeting is canceled.

Manila has decided to let us report at the Dumaguete Immigration office.

    Cheers
