Health & Wellness Physical, Dentist, Lab, Vaccinations

I'm tired of sitting around the US freezing my butt off so I'm coming back to the Philippines until they straighten out my clearance issues.

In the mean time I have a few things to take care of to prepare for my deployment but I am not familiar with any doctors and labs capable or competent enough to fill out these forms.

If anyone has any idea of a good general practitioner, a good dentist and where I might get these vaccines I would sure like to know. Any information would be greatly appreciated.

Right......this forum is not compatible with Android 4.4. Can't load the pictures of my forms. So I'll just say I need a DD2808 (Military Enlistment Physical) and DD2813 (Military Dental form for deployment) completed. I need a Urinalysis for drugs and alcohol and an HIV test. I think I can get all these in dumaguete.

Vaccines include Hep A/B, H1N1, Typhoid, MMR, TD, Flu, PPD, Smallpox, Anthrax, and Vericella.

Required Labs are HIV, G6PD, DNA, U/A, CMP, CBC, and Lipids. Lab and vaccines might be a trick in the Philippines.

Any help would be appreciated.
