How does one revoke a "Special Power of Attorney, when the attorney does not provide a copy of the original document, date of the document and other information required to revoke.

The Register of Deeds should have a copy if the document was notarized.

The Register of Deeds office requires the information only found on the copy before they can locate the document.

A Special Power of Attorney can be revoked by a more recent document so one should make a new document to supercede and revoke an older one. The new one should be worded in such a way that it revokes any past document pertaining to the case. This would apply to the one giving the power and not by anyone else.
