    Hello all, as the Department of Foreign Affairs's web link is not working, I appeal for help in obtaining a Filipino Passport Application Form in a printable format (i.e. adobe acrobat) as from the Department of Foreign Affairs ( Region VII- RCO Cebu) at Cebu City.
    That would represent a big helping hand for my nephew whom is applying for his first passport.

    go to philippine embassy washington dc website

    union jack
    try going to philippine embassy of washington dc website and selecting downloadable forms. the passport application is right there. from my experience, obtaining passport can be overwhelming and you may wish to use an agency. good luck jim

    Hi all, excuse my thickness, could you tell me if any application form would be considered valid to apply for a Filipino Passport? I'm asking because I know that there are different regions' application forms with the name of the region of origin (i.e. Manila or Cebu) printed on the letterhead. The form is not for myself but for my nephew who is a filipino national residing in Dumaguete and he has to travel to Cebu to apply for his first passport. I know the local authorities can be a little awkward at times and they might make a fuss over the origin of the application form.........:(

    With due respect of Coyote's attempt to assist, I suspect that Union Jack may raise a valid point.

    This one is from the Department of Foreign Affairs own website here in the Philippines. It "might" be closer to the mark.

    Good luck. Please let us know how you fare. I'll probably be just behind you in line with a family member in tow in the not too distant future.

    We obtained a passport for my Brother-in-law. He obtained his from the office in Cebu, even though the Dumaguete office can assist, physical presence will be required in Cebu and it will take several trips or visits to the office. There are just too many people getting passports through a tedious process to get everything done in one day. We obtained the form from the Cebu office.

    To Pedro...could you remember if on the application's letterhead it had printed Cebu?
    I've got a form with Manila on the letterhead which I now attach, just wondering if we are talking about the same form or not.

    I only provided fianacial support, I never got involved in the filliing out of the form. I would hope that except for the city the form originated from there is no other difference. I would not obsess about it. What I can tell you though is that the in country applications differ considerably from the ones issued at the foreign PI embassies. My wife had to renew hers while in the US and it was completely different. My guess would be that the embassies are given more resources than the in country DFA offices get so the form while asking for the same information is more simplified than the ones from the embassies.

    OK, after looking at another version of the application form, and concluded that the difference is neglectible, I can now say that the one I've attached in my previous message is a valid one. As all links to the DFA seem not to work, feel free to download it if needed.
