I know from your other postings here that you have medical awareness - but looking online (which I am sure you have done, so apologies for repetition):

    * Hernias can often be diagnosed based on signs and symptoms. Occasionally, medical imaging is used to confirm the diagnosis or rule out other possible causes. The diagnosis of hiatus hernias is often by endoscopy.

So perhaps a different form of internal inspection is required, other than ultra-sound. Not sure which specialist would be suitable for hernias as they can occur in many places - I checked a thread I posted here last year, based on members' recommendations, but found no specific reference to hernias. However, @jim787 wrote "for Internal Medicine at Silliman: Dr. Dr. Junabeth Credo treated me successfully and non-intrusively for gallstones and I have gone to her since for yearly examination. She seems to "know everything" and to keep up with specialties better than some specialists, and she sometimes teaches in the med school."

Also, Dr Henry Tan at Holy Child is a doctor I have great faith in (based on the fact I can often tell whether or not they talk rubbish (he talked pure 100% sense) and how he treated a relative after a poor treatment elsewhere and at 1/16th the cost!) - he may not work in the field of hernias but would almost certainly be able to point you in the right direction.

I know none of this helps with the finances - the first issue, but surely the Barangay must be a port of call.
