Anyone ever had a DD Form 2813 completed/filled out by a dentist in Dumaguete? I'm considering taking the form to Dr. Ong to have him/her(?) do the checkup for my CRC prerequisite. My company doesn't really see a problem with a Filipino dentist filing out this form and I personally don't think the government will even check/care. If there are problems I would much rather have it taken care of here or even "talk" my way to a signature on the form than spending thousands of dollars in the US to have any issues fixed. Thoughts?
