Just because a doctor is billed at a certain rate does not mean they are getting it all. Based on the information below for Manila, I would guess that the doctor is getting substantially less than 885K. It may be the hospital taking a big piece of the fee. While salaries maybe less, I would guess that a new MRI system costs about the same regardless of where it's purchased, so fixed costs aren't so different in the Philippines, which might explain some of this.

One reason for the long waits, is the lack of Doctors making appointments, the first come first serve just doesn't work, if they would make appointments, it would run smoother.
They, SUMC, do now have an area for the Heart Center, so far, they can just do limited diagnosis. I toured the area since I am a cardio patient, and yes they are planning to open a surgery in the future. The problem I see is getting cardiovascular surgeons on staff. Once that happens, then more procedures can be added. You simply cannot offer certain procedures without a certified surgeon on staff in case something goes wrong.-

The stainless steel insert cost 125K Peso. Apart from that you obviously get charged for the procedure itself (surgeon. use of the operating theater, room, etc, etc. However, as all went as smooth as it did, I'm the last one complaining about the cost. Also got some rebate because I'm (partly) covered by PhilHealth. Keep in mind that the insert (when needed) is to be paid cash in full upon delivery. Hospital bills can be settled by either cash or credit card.
